About Unity Home Health Care

Unity Home Health Care focuses on helping to maintain optimal physical, mental, and psychosocial function. We focus on the client's potential, not their limitations. Helping clients do things for themselves rather than "doing it for them" helps the client achieve short and long term goals. This can help to improve the client's self-image and self-esteem. Some example goals of Unity Home Health Care are to promote activity, mobility, and independence with daily living, increase muscle strength and balance and prevent contractures and pressure ulcers. Every person can benefit from Unity Home Health Care. When there is a need for hospice care, we work in conjunction with hospice agencies to provide the best care needed. 

Unity Home Health Care provides Nurse Managed Care, Certified Nursing Assistants and Companions , depending on the level of care. C.N.A.'s have been trained to safely perform complex and important tasks. They are trained to watch for and report changes in their clients' condition to the Nurse Care Manager.
